The following are the required forms and other paperwork that you must submit with your grant application.
Please note that the forms are available in PDF and Word. PDF is suitable for printing and filling in by hand or with a typewriter. Downloading these files requires the free Adobe Acrobat Reader. If you don't have it, you can get it here. Word templates are suitable for downloading and filling in by typing and using the mouse on your computer. To use these files, you must have Microsoft Word installed on your computer. If necessary, you may use these directions to unlock the templates and add material such as images. Checklist for Participants in a Cooperative Project (LG-PA) - needed only if your project involves more than one local government.

Vendor Quote Form (LG-VQ). The form is required in the following cases:
- In each instance where funds are requested for supplies and materials or equipment with a unit cost in excess of $5,000
- For purchased services, where the cost of a vendor's services, including a consultant's services, exceeds $5,000
- Remodeling, where the cost of any one activity exceeds $5,000 or where any one vendor will receive over $5,000. If more than one contractor is used, this form must be completed for each.
